TSINY, a mental health services agency is seeking Case Managers for its Mobile Outreach Program located Queens (Jamaica), NY.

Annual Salary: $19.78, Full-Time

Position Summary

The MOP Case Manager works as a member of a team to provide case management and counseling to individuals, assess needs, make referrals, link individuals to services, follow up with discharged individuals, assist with transportation to appointments, participate in staff training, and maintain case records in accordance with DOHMH regulations and TSINY policy and procedure.

Essential Functions

  •      Contact potential participants and referrals within 24 hours of receipt of the referral.
  •      Assess the needs of the participant upon enrollment to determine current needs and desired life goals.
  •      Inform participants of available mental health resources in the community.
  •      Complete a brief individual service plan with participants.
  •      Meet at a minimum, weekly, with enrolled participants.
  •      Provide case management to participants.
  •      Make referrals and link participants to services & follow-up to ensure that the actual linkage has been made.
  •      Refer participants to programs which offer long-term services such as obtaining benefits/entitlements and supportive housing.
  •      Remain in direct communication with the participant and collaterals to facilitate coordination of care during the 6-8-week engagement period.
  •      Facilitate discharge planning with the participants & follow up with the participants 30 days’ post discharge.
  •      Conduct outreach efforts to participants who have been disengaged from services.
  •      Conduct outreach activity at least twice per month to educate the public about mental health, support services and increase program enrollment.
  •      Maintain case records in a timely manner and in accordance with DOHMH regulations and TSINY policy and procedure.
  •      Complete all necessary paperwork, statistics, and correspondence in accordance with program and Agency guidelines.
  •      Escort participants to appointments, as needed.
  •      Attend and participate in monthly staff meetings.
  •      Participate in weekly individual supervision.
  •      Participate in staff training and program development meetings.
  •      Be able to sit or stand as needed, with or without reasonable accommodation.
  •      Be able to walk, primarily on a level service, for short periods throughout the day, with or without reasonable accommodation.
  •      Be able to reach above shoulder height, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.
  •      During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
  •      Perform other related duties as required.

Experience/Education/SkilIs/Abilities

Bachelor’s degree or Associate degree plus one years’ experience or High School diploma plus two years’ experience in mental health setting.
