Transitional Services for New York, Inc. (TSINY) is a comprehensive, community-based, mental health agency located in New York. We provide a continuum of rehabilitative services to enrich the lives of people with mental illness and facilitate their transition to increased levels of independence.

Transitional Services accepts referrals from hospitals, physicians, mental health professionals, families and friends, employers, schools, employee-assistance programs and individuals themselves.


Fact: Severe mental illnesses are more common than cancer, diabetes or heart disease.


Fact: Mental illnesses are biologically-based, no-fault brain disorders that often can be successfully treated.

One in four adults – approximately 61.5 million Americans – experiences mental illness in a given year. One in 17 – about 13.6 million – live with a serious mental illness, such as schizophrenia, major depression or bipolar disorder.

TSINY was established in 1975 as an alternative to long-term hospital placement. TSINY has since expanded into a multifaceted mental health organization offering the full range of residential, case management, rehabilitation and recovery supports assisting people with serious mental illness access permanent housing, increase personal motivation, pursue employment, pursue education, and meaningfully participate in their
communities.


TSINY Program Outlines


Supervised Living Settings
Group living situations with 24-hour support staff to assist residents in all daily living skills. Programs are available in Queens and the Bronx.

Apartment Treatment Program
Residents live with roommates in apartments located throughout the Queens community. Support services are provided according to individual needs.

Supported Housing
Provides rental subsidy and housing-based case management to tenants living in their own apartments in the community. Tenants live either alone, with roommates or families. Our apartments are located throughout Queens and Nassau Counties.

Miele’s Respite
A crisis respite center providing a soft landing to people who are experiencing a psychiatric or emotional crisis as an alternative to hospitalization
for up to 28 days.

Crisis & Counseling Center
The Crisis & Counseling Center is an OMH licensed outpatient mental health clinic, located in Jamaica, providing short-term treatment, ongoing support and crisis care to alleviate the symptoms of mental illnesses and improve coping skills.

Educational and Employment Programs
The educational program aids in developing literacy, GED preparation, college education and technical training. Employment Services offer evaluation, employment readiness training, job placement assistance and ongoing supports during employment.

Our affirmative business, Turn the Page…Again, a bookstore located in Queens, offers a variety of competitive employment positions and opportunities to prepare individuals for more permanent employment opportunities.

Personalized Recovery Oriented Services (PROS)
PROS is a comprehensive recovery-oriented program integrating evidence-based practices, clinical treatment, rehabilitation & recovery support in a person-centered environment. Services are rooted in a strengths-based approach teaching people to use their inherent strengths and natural supports to overcome barriers caused by their mental health condition that may prevent the achievement of their personal goals.

Mobile Outreach Program
Mobile Outreach offers temporary case management services while working on linking individuals to a long-term treatment provider of their choice.

The Behavioral Health Care Coordination
The BHCC program delivers consumerdirected, recovery-oriented care coordination and services to increase the individual’s capacity to manage their health and wellness, live a self-directed life and reach their full potential.

Adult Protective Services (APS)
APS provides state-mandated services to individuals suffering from mental and/or physical impairments who are unable to manage their own resources carry out the activities of daily living or protect themselves from abuse, neglect, exploitation or other hazardous situations and have no one available and willing to assist them. Services are designed to prevent or eliminate risk factors by strengthening an individual’s capacity to function and to self-direct.

Empowerment Center
The Empowerment Center is a peer-run program providing range of individual and group peer services designed to engage individuals around three social determinants of health – economic stability, social inclusion and health promotion. Center members expand their capacity to manage their own health, wellness and recovery.

CORE & HCBS Services
CORE & HCBS Services provide opportunities for eligible adult Medicaid beneficiaries with serious mental illness to receive services in their own home or in the community. Services include Peer Supports, Education Supports, Employment Support, Rehabilitation and Recovery Supports and Family Support.
