Our Centralized Intake, Assessment, Discharge and Client Finance (IADF) department seeks to match everyone with the appropriate level of support from TSINY to pursue their recovery goals. Our intake staff process referrals for all our agency programs and provide quick access to services. Our staff will assist you in exploring the service offerings at TSINY that include Case Management, Mobile Crisis Services, Crisis Respite, Housing Supports, Home & Community Based Services, Peer Supports, Employment Services and Educational Supports.
Our staff will schedule an intake appointment with you, often within 24 hours. Our staff will also help you understand any supporting documentation you will need before starting services that may include:
- Photo Identification
- Proof of Income
- Proof of Insurance
- HRA 2010e Housing Application (For Housing Programs)
- Recent Medical Exam
- Mental Status Exam
TSINY will accept referrals from individuals, providers, community partners, and single point of access programs.
