Case Manager

Manages own caseload and related records; attends staff meetings and training. Bachelor’s, or Associate’s degree plus 1 year’s experience or HS diploma plus 2 years’ experience in mental health setting, and a valid NYS driver’s license with a clean record.

HS diploma or a combination of education and relevant experience to total four years and a graduate of a Peer Advocate training program.  Has a lived experience with mental health services.

Responsible for assisting individuals with serious mental illness to remain in stable housing in the community in a safe environment. Will report and work directly with the Program Director to develop program policy and procedure, as well as monitor the program’s ongoing regulatory compliance.  Bachelor’s degree, with 3-5 years prior related experience and valid/clean NYS Driver’s license
