The Senior Case Manager coordinates and monitors the assessment, treatment planning and care coordination for each resident on their caseload.  The Senior Case Manager serves as an advocate and intermediary for and on behalf of resident’s rights. The Senior Case Manager will work in partnership with our residents to establish goals, find appropriate supports and access independent community housing.

Driver’s license required.

Bachelor’s degree plus one year’s experience, Associate’s degree plus two year’s experience or High School diploma plus three year’s experience in mental health setting.
