The Peer Health Navigator (PHN) is a full-time position. Responsible for working with BHCC consumers in developing a plan to support recovery and effectively manage future crises as they occur. The PHN will facilitate linkages to available community-based supports that address physical health, behavioral health and social determinants of health. The PHN will work as part of the BHCC team who provide intentional peer support to individuals experiencing behavioral health crises.
Perform other related duties as required.
Should have at least a high school diploma, or equivalent, and have NYS Peer Certification
Minimum qualifications may be waived in unusual circumstances with approval of the CEO as long as City and State requirements are met.
