Provide care coordination and health promotion to individuals throughout Queens
Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care
Assist individuals in obtaining Food Stamps and Social Security benefits
Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy
Manage a specialized caseload SMI individuals and assist with other duties as needed
Completes all required documentation according to standards and no later than 24 hours of service delivery

Must have 2 COVID Vaccinations
