Case Managers $43,334

The Case Manager coordinates and monitors the assessment, treatment planning and care coordination for
each resident on their caseload. The Case Manager serves as an advocate and intermediary for and on
behalf of residents’ rights. Case Management staff will work in partnership with our residents to establish
goals, find appropriate supports and access independent community housing.

All applicants must be fully vaccinated.

Manages own caseload and related records; attends staff meetings and training. Bachelor’s, or Associate’s degree plus 1 year’s experience or HS diploma plus 2 years’ experience in mental health setting, and a valid NYS driver’s license with a clean record.
