The Adult Home Care Coordination Supervisor (Supervisor) provides clinical and administrative supervision to Adult Home Care Coordinators who are providing intensive care management services to individuals identified as Adult Home (AH) class members under the Stipulation and Order of Settlement, United States v. State of New York. The Supervisor oversees the care coordination, care planning, and other activities of the Adult Home Care Coordinator. The Supervisor also ensures that Adult Home Care Coordinators address unmet needs that impact on the transition from the adult home into the community.
Licensed level healthcare professional (e.g., RN, licensed clinician, psychologist) with prior experience in a behavioral health clinic or care management supervisory role; or Master’s level professional with three (3) years prior experience supervising clinicians and/or care managers who are providing direct care services to individuals with Serious Mental Illness (SMI) or Substance Use Disorder (SUD)
