Mobile Outreach Program Case Manager

$36,000 annual

The MOP Case Manager works as a member of a team to provide case management and counseling to individuals, assess needs, make referrals, link individuals to services, follow up with discharged individuals, assist with transportation to appointments, participate in staff training and maintain case records in accordance with DOHMH regulations and TSINY policy and procedures.

Bachelor’s degree or Associate degree plus one year’s’ experience or High School diploma plus two years’ experience in mental health setting.

The Adult Home Care Coordination Supervisor (Supervisor) provides clinical and administrative supervision to Adult Home Care Coordinators who are providing intensive care management services to individuals identified as Adult Home (AH) class members under the Stipulation and Order of Settlement, United States v. State of New York. The Supervisor oversees the care coordination, care planning, and other activities of the Adult Home Care Coordinator. The Supervisor also ensures that Adult Home Care Coordinators address unmet needs that impact on the transition from the adult home into the community.

Licensed level healthcare professional (e.g., RN, licensed clinician, psychologist) with prior experience in a behavioral health clinic or care management supervisory role; or Master’s level professional with three (3) years prior experience supervising clinicians and/or care managers who are providing direct care services to individuals with Serious Mental Illness (SMI) or Substance Use Disorder (SUD)

$41 per hour

Work as a member of a team to reduce the number of psychiatric hospitalizations among individuals with a history of severe and persistent mental illness by administering injectable psychotropic medications and conducting medication management groups. In addition, address the high prevalence of medical illness among our clients by providing health monitoring services, nutritional counseling and education, medical education, wellness programming, and following up on outstanding health concerns.

Responsible for determining eligibility and providing protective services to APS eligible adults.  Responsible for assessing eligibility, developing protective service plans, coordinating service plan implementation and coordinating services for each case assigned to them.  Develop partnerships with eligible adults to establish goals, find appropriate supports and live safely in the community.

BA degree in gerontology, social work, psychology, or a related social service area; plus 3 years’ experience working with the elderly, individuals with mental illness or resistant clients.

Reports to the Director of the Crisis Respite Center (CRC) provides services directly to the CRC guests. Completes the Intentional Peer Support (IPS), Need Adapted Treatment Model (NATM) and Peer Health Navigation (PHN) trainings provided as part of the Parachute NYC grant & will practice according to the principles of recovery oriented & trauma informed service delivery including IPS, NATM & PHN. Exp. as a recipient of mental health services & graduate of a core peer training program preferred.
